What is Collaborative Practice?
Collaborative Practice is a nationally recognised programme, which aims to develop a more holistic and patient focused approach to health care, by providing support for non-urgent or non-medical needs with the help of Practice Champions.
Practice Champions are volunteers who give their time alongside the Practice Team to offer social support for patients through activities and building connections to local community groups, and finding new ways to improve the services that the Practice offers
