LMN Primary Care Network
LMN Primary Care Network is a collaboration of practices joining up services to offer a range of diverse clinics to the population in Meopham and Longfield. The practices covered in our PCN are Kent House Surgery, New Ash Green Surgery, and Meopham Medical Centre.
Since the NHS was created in 1948, the population has grown and people are living longer. Many people are living with long term conditions such as diabetes and heart disease or suffer with mental health issues and may need to access their local health services more often.
To meet these needs, GP practices are working together with community, mental health, social care, pharmacy, hospital and voluntary services in their local areas to enable greater provision of proactive, personalised, coordinated and more integrated health and social care for people close to home.
You can access PCN services such as your Mental Health Nurse, Children's and Young Persons's Social Prescribing Link Worker, Pharmacist, Care Co-ordinators and Paramedics by phoning your local GP surgery and asking for more information.
